PLEASE READ THESE TERMS OF SERVICE CAREFULLY BEFORE CREATING AN ACCOUNT, ACCESSING, OR USING THE CALABASAS LONGEVITY PLATFORM. BY CREATING AN ACCOUNT OR USING THE PLATFORM, YOU AGREE TO BE BOUND BY THESE TERMS IN THEIR ENTIRETY.
CALABASAS LONGEVITY IS NOT A MEDICAL PRACTICE, PHYSICIAN GROUP, HOSPITAL, CLINIC, HEALTHCARE FACILITY, HEALTHCARE PROVIDER, PHARMACY, DRUG MANUFACTURER, DRUG DISTRIBUTOR, OR TELEMEDICINE PROVIDER.
Calabasas Longevity is a health and wellness platform that provides non-clinical services, educational content, wellness resources, referral facilitation, administrative support, and communication tools. All clinical services are provided exclusively by independent licensed healthcare providers affiliated with GuardianMD and its affiliated professional entities. Technology, payment processing, platform administration, and pharmacy coordination services may be provided by Asher Med, LLC and its affiliates.
Nothing on the Calabasas Longevity website, platform, or communications constitutes medical advice, a diagnosis, or a treatment recommendation. All content is provided for educational and informational purposes only. Always seek the advice of a qualified licensed healthcare provider for any medical questions.
Calabasas Longevity makes no representation that any user will qualify for any treatment program, that any physician will prescribe any medication, or that any particular health outcome will be achieved. All clinical determinations are made exclusively by the licensed healthcare provider who evaluates the user.
Calabasas Longevity provides only non-clinical services. Clinical services, including telemedicine consultations, clinical evaluations, and prescriptions, are provided solely by licensed healthcare providers affiliated with GuardianMD. Pharmacy fulfillment is provided solely by licensed third-party pharmacies. Calabasas Longevity exercises no control over clinical decision-making, prescribing practices, or pharmacy dispensing.
Users may create a free account without enrolling in a paid program. Free accounts provide access to educational content, program information, and basic platform navigation. Free accounts do not include access to clinical services, physician consultations, or prescription services.
Users who wish to access clinical services and physician consultations must enroll in a paid program. Program terms, including fees and billing, are disclosed at enrollment.
Membership services may include enhanced access to educational content, wellness resources, facilitated access to independent GuardianMD providers, administrative support, and communication tools. Membership fees are entirely separate from clinical decision-making. Payment of any fee does not guarantee any clinical service, prescription, treatment, or health outcome.
Clinical services available through the platform are provided by licensed physicians affiliated with GuardianMD and its affiliated professional entities. GuardianMD Providers are independent of Calabasas Longevity and are solely responsible for all clinical decisions, prescribing decisions, and medical care rendered. No GuardianMD Provider is an employee, agent, or representative of Calabasas Longevity.
A separate $20.00 physician consultation fee is charged directly by GuardianMD or its billing entity, KinBri LLC, at the time of clinical intake. This fee will appear as a separate charge on your payment statement and is NOT processed through Calabasas Longevity's merchant account. This fee is mandatory and non-refundable regardless of whether a prescription is approved or denied, as it compensates the independent licensed physician for their clinical evaluation services.
CALABASAS LONGEVITY IS NOT A PHARMACY.
Calabasas Longevity does not compound, manufacture, dispense, ship, or sell any medications. All prescriptions, if issued by a GuardianMD provider, are fulfilled exclusively by licensed third-party 503A compounding pharmacies. Calabasas Longevity is not responsible for prescription fulfillment timelines, shipping delays, pharmacy errors, drug recalls, or any harm arising from the use of any medication.
Some medications may be compounded medications. Compounded medications are not FDA-approved in their compounded form. All risks and benefits of compounded medications should be discussed with your GuardianMD provider prior to use.
Program fees charged by Calabasas Longevity are composite charges that may include components attributable to: (a) platform and wellness services; (b) physician coordination; and (c) pharmacy fulfillment fees when a prescription is issued. All applicable fees are disclosed at enrollment.
If a GuardianMD provider determines you are not a suitable candidate for the requested treatment program, Calabasas Longevity will issue a full refund of your program fee. Once a prescription has been processed and fulfilled by the pharmacy, no refund is available as the medication has been compounded specifically for you. Please note that the $20.00 GuardianMD physician consultation fee is non-refundable regardless of outcome, as it compensates the independent licensed physician for their clinical evaluation.

By enrolling in a paid program, you authorize Calabasas Longevity and its designated payment processing partners to charge the payment method provided for all applicable fees. You represent that you are authorized to use the payment method provided.
THE CALABASAS LONGEVITY PLATFORM IS NOT DESIGNED FOR MEDICAL EMERGENCIES. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 IMMEDIATELY OR GO TO THE NEAREST EMERGENCY ROOM.
Calabasas Longevity's collection and use of personal information is governed by our Privacy Policy, available at calabasaslongevity.com/privacy. By using the platform, you consent to our privacy practices as described therein.
THE PLATFORM AND ALL SERVICES ARE PROVIDED ON AN "AS IS" AND "AS AVAILABLE" BASIS WITHOUT WARRANTY OF ANY KIND. CALABASAS LONGEVITY EXPRESSLY DISCLAIMS ALL WARRANTIES, WHETHER EXPRESS, IMPLIED, OR STATUTORY, INCLUDING WITHOUT LIMITATION ANY IMPLIED WARRANTY OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT.
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, CALABASAS LONGEVITY'S TOTAL AGGREGATE LIABILITY TO ANY USER SHALL NOT EXCEED THE MEMBERSHIP SERVICES FEES ACTUALLY RETAINED BY CALABASAS LONGEVITY DURING THE TWELVE MONTHS PRECEDING THE EVENT GIVING RISE TO THE CLAIM, OR ONE HUNDRED DOLLARS ($100.00), WHICHEVER IS GREATER. CALABASAS LONGEVITY IS NOT RESPONSIBLE FOR THE CLINICAL ACTS OR OMISSIONS OF ANY GUARDIANMD PROVIDER OR ANY PHARMACY.
All disputes arising out of or relating to these Terms shall be resolved through binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, in Los Angeles County, California. Class actions and class arbitrations are not permitted. These Terms are governed by the laws of the State of California.
